What the inquiry form collects
The form requires your name, work email, company, and a description of the problem. Phone number, current process, frequency, business impact, and timeframe are optional. Please provide only the context needed to evaluate the inquiry. Do not include credentials, patient records, financial account details, or confidential files.
Where inquiry information goes and why
Submitted information is stored in a database supporting this site and displayed in an owner-restricted inbox for Andrew to review. It is used to understand the problem and respond about a potential engagement. The form does not automatically add you to a marketing list or publish your information.
The site uses Sites hosting and Cloudflare-backed infrastructure to process and store submissions. If you choose to email directly, that message is handled through the email services used by you and CTQ One. Site-inbox submissions do not currently generate email notifications.
Optional website analytics
When analytics is configured, CTQ One uses Google Analytics 4 only after you choose to allow analytics. It helps measure page use, traffic sources, important calls to action, and whether the inquiry process is completed. Declining analytics prevents the Google Analytics script from loading on this site.
Analytics may process page URLs, page titles, referrer and campaign information, device and browser details, approximate location derived from network information, and interaction events. CTQ One does not send inquiry answers, names, email addresses, phone numbers, company names, or inquiry reference numbers to analytics.
Your choice is stored in this browser. You can reopen Privacy choices from the control shown after you make a selection. Analytics data retention is intended to be set to 14 months, with advertising signals and ad personalization disabled initially.
Abuse prevention and technical processing
The hosting infrastructure processes technical request information as needed to serve and protect the site. The inquiry endpoint uses short-lived hashed request and email rate-limit keys to reduce abuse. It does not store raw IP addresses in the inquiry table. Expired rate-limit records are removed during subsequent submission attempts.
Owner sign-in uses the hosting platform authentication system. The form includes a hidden spam field and server-side validation.
Questions, corrections, and removal requests
Inquiries remain stored until removed by the site operator; automatic retention deletion is not configured. To request a correction or removal, email andrew@ctqone.com and include enough information to locate your inquiry, preferably its reference number. We may need to verify that the request concerns your information.
This form is an initial contact channel. Any confidentiality, service, or data-handling terms for a paid engagement are addressed separately before that work begins.
